WHAT TO EXPECT IN A SESSION
Sessions are conversational, collaborative, and tailored to the client's unique needs.
We’ll talk about what’s going well, where you’re feeling stuck, and what’s happening in your life that may be impacting your health. Together, we’ll identify realistic strategies and small, meaningful practices to focus on between sessions. The sessions are without judgment, pressure, or a one-size-fits-all approach.
Meet your Dietician, Sarah Hammaker, RDN, LDN
Sarah Hammaker, RDN, LDN, is a Registered Dietitian and founder of Practice Health, where she helps people build a healthier relationship with food, movement, and their bodies. With more than 20 years of experience, Sarah takes a holistic, individualized approach to nutrition. She looks beyond what someone eats and coaches on the 5 Pillars of Health—Nutrition, Movement, Hydration, Sleep, and Stress Management.
Her work is rooted in the belief that health doesn’t have to be complicated or perfect to be meaningful. Rather than following rigid rules or believing in quick fixes, Sarah helps clients come back to the basics, understand what their bodies need, and create sustainable practices that support long-term well-being.

People work with a dietitian for all kinds of reasons. I support clients with weight and body composition goals, diabetes, heart and digestive health, menopause and midlife nutrition, sports nutrition, disordered eating, and general wellness.
You don’t need a specific diagnosis or health concern—sometimes you simply want to feel better, have more energy, better understand your body, or feel more confident about how you’re fueling it.

Sometimes meal ideas or more structured guidance can be helpful, but my goal isn’t to hand you a rigid plan to follow.
We’ll work together to find realistic ways of eating that meet your needs and fit your actual life.

My approach is holistic, individualized, and grounded in evidence-based nutrition. Rather than focusing on food in isolation, we’ll look at the bigger picture, including movement, sleep, stress, mindset, and your current season of life.
I especially enjoy working with adults navigating midlife and menopause, metabolic health, body composition, and developing a more balanced and sustainable approach to health.

Yes. I am in-network with several major insurance plans, and many plans cover nutrition counseling at little or no out-of-pocket cost. Coverage varies by plan, so we can verify your benefits before your first appointment.

That depends on your needs and goals. Many clients begin by meeting every two to four weeks, and we can adjust the frequency as you gain confidence and find what works for you.
